Increasing Engagement with Community Exercise Programs in Plattsburgh, NY

Abstract
Falls are a major cause of morbidity in older adults and are associated with significant public health costs. Exercise can improve strength, balance, mood, and social connection while helping reduce fall risk, I developed and implemented an Epic Smartphrase to provide older adult patients in Plattsburgh, New York, with information about local community exercise programs and at-home exercise resources. Used in post-visit instructions, the smartphrase includes practical details such as eligibility, cost, and contact information to make counseling more actionable during routine care. This project demonstrates a simple EMR-based intervention to connect patients with community exercise resources.
