Contextualizing Advanced Directives for Patients with Medical Literature and Data

Abstract
End-of-life care for older adults in the U.S. often exacts an emotional, social, and economic cost that is often misaligned with patients’ values. Tool such as advanced directives that can better communicate patients’ wishes in the event that they become incapacitated are underutilized and present questions about topics (cardiac resuscitation, mechanical ventilation, artificial feeding, end-of-life antibiotics) about which patients tend to have an incomplete understanding. Physicians face a variety of barriers to helping patients fill out advanced directives, including poor or non-reimbursement, time pressures, and patient unwillingness to discuss what can be a sensitive topic. The following presents a handout that can be given to patients in physical or electronic form that acts as an aid in educating patients about what advanced directives are and why they important to have while also giving information and context about the interventions they are asked to make decisions about.
