Whose Ambulance Is It, Anyway? A National Analysis of Emergency Medical Services System Delivery and Response times

Abstract
Emergency medical services (EMS) systems in the United States (US) rely on various delivery models, including fire-based, public, hospital-based, private, and tribal agencies. The effectiveness and efficiency of these different models is key in guiding EMS system decision-makers when presented with the option to implement a new model or restructure. Response time is a reliable and objective measure that, while not sufficient, can serve as a proxy measure for EMS system performance. To date, no studies have examined the swiftness or consistency of EMS response times as they vary by system type. This study aimed to compare different EMS delivery models by response time and correlate this variation with clinical patient outcomes. The National EMS Information System (NEMSIS) Public Research Datasets for the years 2017-2024 were obtained and complete records for transport ambulance responses to emergency calls were included (n = ~93,388,000).
