Improving Patient Understanding of Cholesterol-Lowering Diets Through a Clinical Handout

Abstract
This community health project focused on improving patient understanding and memory of cholesterol lowering diets through the development of a simple, practical educational handout for patients with dyslipidemia. Observations in Nuvance Health Primary Care—New Canaan revealed that many patients understood the importance of lowering LDL cholesterol but struggled to translate general dietary advice into actionable lifestyle changes. To address this gap, informal patient discussions and interviews with registered dietitians were used to begin creating a simple handout that contained effective dietary strategies and practical “swap this for that” recommendations that could be incorporated into patient wellness checks and follow-up visits. The project aimed to supplement provider counseling with an easy to reference resource that could improve patient confidence, reinforce lifestyle modifications, and potentially reduce cardiovascular risk through better dietary adherence.
