Patient Attitudes Toward GLP-1 Receptor Agonists

Abstract
The rapid rise in use of glucagon-like peptide-1 (GLP-1) receptor agonists for weight loss has been accompanied by significant public enthusiasm, driven in part by social media, celebrity endorsements, and expanding direct-to-consumer access. However, the relationship between patient enthusiasm and clinical outcomes remains unclear. This study aims to evaluate whether patient-reported enthusiasm for weight loss while using GLP-1 agonist therapy correlates with self-reported weight loss and engagement in lifestyle modifications. We will conduct a cross-sectional survey of adult patients receiving GLP-1 agonist therapy in primary care settings. Participants will be recruited through participating clinics and complete a de-identified questionnaire administered via REDCap or paper format. Survey domains will include baseline and post-initiation enthusiasm for weight loss, lifestyle behavior changes, perceived drivers of enthusiasm, sources of information about GLP-1 therapy, and methods of medication access (e.g., primary care vs direct-to-consumer platforms). We hypothesize that greater enthusiasm for weight loss will be associated with increased adoption of lifestyle changes and improved self-reported weight outcomes. Additionally, we will examine how heightened public attention and advertising influence patient interest, initiation of therapy, and engagement with primary care providers. This study addresses a critical gap in understanding the behavioral and psychosocial factors influencing GLP-1 treatment outcomes. Findings may inform how clinicians counsel patients, particularly in an era of widespread direct-to-consumer marketing and evolving access pathways. Ultimately, clarifying the role of patient motivation and engagement may help optimize the effectiveness of GLP-1 therapies and improve long-term cardiometabolic outcomes.
