Abstract
Assisted living and long-term care planning remain deeply misunderstood topics for many families, often avoided until a health crisis; such as a fall, hospitalization, or sudden cognitive declines. As a community health project completed in Newtown, CT, this initiative sought to address that gap by creating a plain-language, Connecticut-specific educational brochure. Interviews conducted with clinical staff at Nuvance Health revealed consistent themes: families frequently arrive underprepared, are confused by the differences between assisted living, memory care, skilled nursing, palliative care, and hospice, and are blindsided by the financial realities. The brochure directly addresses these gaps by providing a side-by-side comparison of all five care types, a step-by-step guide to the placement process, Connecticut-specific paperwork requirements, and a curated list of free local resources.
