Abstract
Prioritizing vaccines and screenings, especially in communities with high-risk behaviors and exposures, not only minimizes the overall financial burden on the healthcare system but also prioritizes preserving patients’ well-being and quality of life. Using appropriate preventive guidelines, an intervention was designed to provide a standardized, provider-friendly preventive care protocol and a patient-designated checklist that highlights recommended testing by age and associated risk factors. The goal of this project is to promote patient engagement, improve health literacy in the community, and address inefficiencies in clinical workflows. Ultimately, the project seeks to reduce preventable hospitalizations and long-term healthcare costs while promoting equitable, prevention-focused primary care.
