Exploring Vermont Healthcare Providers’ Perspectives on Changes to Medicaid under Public Law 119-21

Abstract
Background: On July 4, 2025, Public Law 119-21 was passed by Congress and mandated sweeping changes in qualifications for public insurance. States scrambled to comply with new regulations and provide continued coverage for vulnerable populations.
Methods: To understand Vermont providers’ knowledge and perceived patient concerns regarding the Medicaid changes in Public Law 119-21, an electronic survey was conducted. The study team partnered with United Way of Northwest Vermont and disseminated a survey to healthcare providers in Vermont.
Results: The survey was completed by 87 healthcare providers, who expressed that their patients were concerned most about losing Medicaid coverage, losing access to specific medical services, the prices of medications, out-of-pocket expenses, and anxiety surrounding Medicaid-related paperwork. Outpatient providers suggested an individual such as the clinic manager to be the best individual to disseminate information on changes about Medicaid to their practice, while inpatient providers and providers who practice both inpatient and outpatient, most commonly recommended the front desk or registration team. Based on statistical analysis, providers’ educational preferences do not change by practice setting, but patient educational preferences do.
Conclusion: Vermont providers are concerned about the effects of Public Law 119-21 on access to health care and perceive that their patients are similarly concerned. This survey informs the next steps in providing and disseminating information to help Vermonters maintain Medicaid coverage. The Department of Vermont Health Access (DVHA) plans to use this information to design and disseminate educational materials and other resources for healthcare teams and worried patients.
