Abstract

A set of eight surveys/questionnaires from Project EVOLVE (University of Vermont) are provided for use. Three surveys/questionnaires address school demographics and personnel utilization pertaining to inclusive education service delivery and special educator caseload and supervision duties. The remaining five surveys/questionnaires solicit team member perspectives from: (a) school administrators, (b) teachers, (c) special educators, (e) paraprofessionals, and (e) parents of students with disabilities, on a variety of issues pertaining to inclusive education for students with disabilities. In addition, two sample letters for parents and paraprofessionals are included to introduce the the purpose of the surveys/questionnaires.
